The availability, benefits, premiums, provider networks, formularies, eligibility requirements, and other terms of insurance products are established by the applicable insurance carrier and may change.

Information displayed or communicated by us is intended for general informational purposes and does not modify the terms of an insurance policy. The carrier’s official policy, Evidence of Coverage, Summary of Benefits, and other plan documents control.

Submitting a form, requesting information, or speaking with an agent does not create insurance coverage. Coverage becomes effective only after the applicable carrier approves an enrollment or application and confirms the effective date.

Medicare Disclaimer

Elevate Advisors LLC is an independent insurance agency and is not connected with or endorsed by the United States government, the federal Medicare program, or the Centers for Medicare & Medicaid Services.

We do not offer every plan available in every area. Information we provide may be limited to plans offered by insurance carriers with which we are authorized to work. Message Frequency

Message frequency varies depending on your requests and service needs but will generally not exceed 10 messages per month.

Charges

Message and data rates may apply depending on your wireless carrier and service plan. Elevate Advisors LLC is not responsible for charges imposed by your carrier.

Reply STOP to any text message to unsubscribe. You may also revoke consent through any other reasonable method that clearly communicates your desire to stop receiving messages.
