XARELTO is a medication that can treat and prevent blood clots. This may lower the risk of stroke, deep vein thrombosis (DVT), pulmonary embolism (PE), and similar conditions.

Eligibility:
• Do you have insurance covering a portion of the cost of Xarelto?
• Are you being asked to pay more than $85 monthly for Xarelto through your insurance?
Other Requirements:
• Program only available for patients who are taking Xarelto, not valid for any prescription written with off-label dosing.
• This program is not insurance and should not take the place of insurance.
• The cost ($85 or $240 + sales tax if applicable) will not count toward your deductible or cumulative out-of-pocket spend. The cost may not be submitted as a claim for payment to any third-party payer or pharmaceutical patient assistance foundation.
• If you choose you may submit the cost to an FSA or HSA. Check with employer if the cost can be submitted to an HRA.
• Registration begins April 1 each year and refills are available through Dec 31. Terms expire at the end of each calendar year and may change.
• Before you register it is important to know you will be asked to provide personal information.
• Offer may not be used with any other coupon, discount, prescription savings card, free trial or other offer.

Eligibility:
• Each program has different requirements, but many of our
patients come to us who are on Medicare and have a Part D plan and are unable to afford their copays or find themselves in the coverage gap of their prescription plan.
• If your insurance company has denied paying for your prescriptions or you have a high copay or coinsurance responsibility.

3.) RX Assistant
Eligibility:
Its best to call to see if you qualify.
• U.S. citizen or resident
• Does not have insurance or insurance doesn't cover medication.
• On Medicare or in donut hole/coverage gap
